Please follow these steps in order to get a scheduled wellness exam:
- Have all previous records for your pet (from any and all clinics) emailed to [email protected]
- Fill out and submit the "New Client Registration" form below
- Have all previous records for your pet (from any and all clinics) emailed to [email protected]
- Fill out and submit the "New Client Registration" form below
Once your pet's records and New Client Registration form have been received, one of our team members will contact you to schedule an appointment.
Please be advised, a deposit for the cost of the appointment is required at the time of scheduling. This deposit will go towards your scheduled wellness visit but IS FORFEITED if cancelled within 48 hours of scheduled exam or there is a no-show to scheduled visit.
